This is achievable with some materials, and careful adjustment of the graphics to ensure a suitable result.

In the Case of Plastics such as Acrylic the result can be a uniform engraving replicating the graphic Blackest part of the image becoming the deepest area of engraving.

An example here is a Concave engraving 15 mm wide.
Engraved 3 mm deep into 6 mm thick Acrylic
The resulting Dish has a raised nipple in the center reflecting the graphic used to achieve the result.

Timbers produce 3D replications of images and graphics with the added interest of the grain in the timber. The harder grain being more raised than the softer areas.
